•	Provides care for the Adult or Geriatric patients, ages 18 years and over.

Job Title: Physical Therapy Assistant

Summary:
The Physical Therapy Assistant (PTA) is responsible for administering physical therapy intervention as indicated within a written plan of care (POC) and supervised by a licensed Physical Therapist in accordance with the Texas PT Practice Act. Essential Duties and Responsibilities
Patient Care:
Administers physical therapy treatments as appropriate established by the Physical Therapist or written PT POC. 
Performs treatments on patients at scheduled times and ensures patient comfort. 
Instructs and prepares patients for treatments, explaining the process and the expected outcomes. 
Applies physical agents, therapeutic exercises, and ambulatory procedures, and monitors patients during treatments. 
Records patient treatments and progress notes in the electronic health record (EHR). 
Reports treatment results to the supervising Physical Therapist and provides feedback regarding patient progress. 
Assists in patient transfers and transports patients to and from therapy sessions as directed. 

Documentation &amp; Reporting:
Maintains accurate treatment records, including the patient’s full name, medical record number, and date of treatment. 
Reports the effectiveness of treatments and denoted progress via daily notes and to the supervising Physical Therapist as needed. The PTA cannot modify established PT POCs.  
Assists in maintaining appointments, records, and patient charge documentation. 
Educates the patient, family, or caregiver on the treatment plan, exercises, and progress. 

Equipment and Supplies Management:
Monitors the stock levels of physical therapy supplies and orders as needed. 
Ensures the proper maintenance and cleaning of equipment used during treatments. 
Assists in the routine maintenance including appropriate documentation of physical therapy equipment.  

Supervisory and Training Responsibilities:
Assists in the supervision and training of PTA students and support staff. 
Ensures that patients receive high-quality care in accordance with the treatment plan. 
Assists in the coordination of patient care, collaborating with other healthcare providers as necessary.

Other Duties and Requirements:
Maintains patient confidentiality in accordance with HIPAA and other regulatory requirements. 
Adheres to infection control and safety protocols when handling patients or materials. 
Follows all policies and procedures as established by the Physical Therapy Department and hospital. 
Demonstrates a positive and professional demeanor when interacting with patients, families, and staff.

Qualifications
Education and Experience:
Graduate of an accredited Physical Therapy Assistant program. 
Current Texas Physical Therapy Assistant License 
Current BLS (Basic Life Support) certification.

Strong verbal and written communication skills. 
Prior experience working in inpatient, outpatient, and Swing Bed programs is a plus.

Job Summary: As a Speech Pathologist at Crane Memorial Hospital, you will provide direct services to patients including evaluations, individual treatment planning, actual patient treatments, and discharge planning.  The Speech Pathologist focuses on improving speech, language, swallowing, cognitive deficits, and hearing disorders to overcome disabilities from a variety of causes.  The Speech Pathologist participates as a member of the interdisciplinary team providing support and information within the area of Speech Pathology.

Essential Job Functions:

•	Assess and evaluate the impact of available patient information on the assessment and treatment process.

•	Provide care for the Adult or Geriatric patient, ages 18 years and over.

•	Conduct and document a thorough assessment of the patient to determine and diagnose communicative, swallowing, and cognitive deficits.

•	Develop and implement an individualized treatment program in conjunction with the interdisciplinary team based on the initial evaluation and containing achievable functional short- and long-term goals.

•	Comply with evaluation, treatment, and documentation guidelines described in the clinical policies and procedures.

•	Submit patient charges accurately and in a timely manner.

•	Enter a progress note in the patient record following each treatment session.

•	Revise the patient&apos;s treatment program, as needed, based upon patient performance.

•	Provide clear, concise discharge instruction including the course of treatment and discharge recommendations utilizing a prescribed format.

•	Orient patients to program services and treatment procedures appropriate to their needs.  Inform patients of any potential risk during any procedure.

•	Work with clinical staff, patients, and families to maximize patient&apos;s use of functional abilities.

•	Educate / instruct patients and family and/or other caregivers about therapeutic procedures to be continued by the patient at home following discharge.

•	Assist in the referral process when a patient requires additional services following discharge.  Maintain current knowledge of community resources. 

•	Attend and participate in interdisciplinary team conferences regarding patient progress, problems, or needs.

•	Conduct and participate in training interdisciplinary team members on Speech Language Therapy techniques and objectives.

•	Act in compliance with corporate, regulatory, and professional standards and guidelines.

•	Provide E-Stim treatment as ordered by physician and medically indicated.

•	Develop an understanding of responsibilities for recognizing abuse and follow policy for making appropriate referrals/interventions.

•	Participate in the Hazardous Material Program.  Hazards include exposure to blood and body fluids, possible communicable diseases, sharp objects and instruments, assorted chemicals and gases as listed in the Hazardous Materials Program Manual.

•	Complies with established Safety and Patient Safety Program practices.

•	Develop an understanding of responsibilities for participation and participation in Performance Improvement activities.

•	Attend and participate as required in scheduled staff meetings, interdisciplinary team meetings, departmental meetings, in-services, and training programs.

•	Actively participate in continuing education opportunities appropriate to discipline and responsibilities for self-development and to maintain clinical currency.

•	Performs other duties as assigned and required to provide quality care to patients.

Non-technical denials (i.e. coverage termed, non-covered service) should be reviewed for appropriateness. When necessary, patients, employers or insurance companies should be contacted to verify correctness of information filed with paper.
•	Print accounts receivable aging report every 30 days or less. Analyst should check status of all accounts that have aged 60 days or more. 
•	Responsible for managing Charity Care and/or Bad Debt validity and monitoring.

We are currently seeking a detail-oriented and proactive individual to join our team as an Insurance Clerk/Admissions Clerk. In this role, you will be responsible for various administrative tasks related to insurance verification, claims processing, patient admissions, and customer service.

Responsibilities:

Verify insurance coverage and compute patients&apos; benefits accurately.
Follow up on claims filed with payers, including filing adjustments and appeals.
Handle phone and correspondence inquiries from patients and payers professionally and promptly.
Pre-certify patients when necessary and coordinate with the Medical Records Assistant.
Review and address denied claims, making necessary changes and appeals.
Ensure compliance with Texas State Law regarding appeals and denials.
Maintain up-to-date knowledge of billing and collecting regulations and procedures.
Assist with patient admissions, including registration and collection of demographic and insurance information.
Provide patients with necessary forms and information regarding financial assistance options.
Collect co-payments and deductibles during registration/admission.
Reconcile cash drawers and handle daily deposits.
File secondary insurance claims and follow up on workers&apos; compensation claims.
Perform other duties as assigned to support the daily operations of the business office.

Qualifications:

High school diploma or equivalent required.
Previous experience in a healthcare or administrative role preferred.
Strong attention to detail and accuracy in data entry.
Excellent communication and customer service skills.
Ability to multitask and prioritize tasks effectively.
Proficiency in Microsoft Office applications.
Knowledge of insurance verification and claims processing procedures is a plus.
